The fertility question

This is the decision. Direct testosterone therapy signals the body to stop its own production, which reduces sperm production. Enclomiphene works upstream — it blocks estrogen's feedback at the pituitary, raising luteinizing hormone (LH) and follicle-stimulating hormone (FSH) so your testes produce more natural testosterone while preserving sperm production. For a man who wants children now or in the future, that difference is often the deciding factor.

When each makes sense

TRT is the stronger, more direct lever and is the right choice for many men whose family is complete. Enclomiphene is the right choice for men who want to raise testosterone while keeping the door open to fertility, or who prefer an oral option. Monitoring

Both require baseline and follow-up labs. TRT monitoring includes hematocrit — the American Urological Association's Testosterone Deficiency Guideline (Statement 11) directs clinicians to measure hemoglobin and hematocrit before and during treatment — plus estradiol and PSA for men over 40.
