TRT & Fertility — What Men Should Know

How TRT affects fertility

Direct testosterone supplementation signals your body to stop producing its own testosterone, which can reduce sperm production — sometimes to the point of azoospermia (no sperm in the ejaculate). This happens because exogenous testosterone suppresses the LH and FSH signals that drive sperm development. Fertility typically recovers after stopping, but the timeline is unpredictable and full recovery is not guaranteed. If you are planning to conceive, this is a conversation to have before starting.

Fertility-sparing options

Enclomiphene (separate product)

An oral therapy that blocks estrogen feedback at the pituitary, raising LH and FSH to prompt your testes to produce natural testosterone — while preserving sperm production and fertility. Often used as a standalone fertility-sparing option for men with secondary hypogonadism and an intact pituitary axis. Not part of the $149/mo TRT program.

hCG (separate product)

Human chorionic gonadotropin acts as an LH analog, directly stimulating the testes. It is often used alongside TRT to maintain intratesticular testosterone, testicular size, and fertility pathways while on direct testosterone. The AUA notes low-dose hCG can help maintain spermatogenesis for men on exogenous testosterone. A separate standalone product.

How to decide

The right choice depends on your labs, your symptoms, and your plans. If you do not care about fertility, direct TRT remains the most direct and best-studied way to replace testosterone. If you do care, the decision shifts from "replacement" to "preservation," and enclomiphene or hCG become relevant.
